* Calendar: fix SQL error on storing events containing rfc822 email addresses with non-ascii characters eg. "Hugo Müller <hm@test.org>"

egw_cal_user.cal_user_id is now an md5 hash of lowercased raw email address (eg. "hm@test.org" in above example). Full attendee information is now stored in egw_cal_user.cal_user_attendee.
Will allow in a further step also to store attendee information for accounts to eg. answer with correct email to external organizers

fix 1.9.007 update to work with PostgreSQL, deleting all broken events having range_start=0 (not a single recurrence or a broken one with cal_start=0), they are not displayed in regular calendar anyway, but might be synced to CalDAV or eSync clients

database updates: fix whole day event cal_end times and fix wrong recur_interval values. The first fix reduces duplicates on SyncML slowsyncs. The second fix prevents event series from being destroyed when a new exception was created on series created on previous EGw versions. Please update your databases.

Please note: timestamps in egw_cal* tables are in server-time,
tz_id / timezone is only used to (re-)calculate recurrences and to
export in iCals (NOT yet implemented)

- timezone data is imported from SQLite DB from Thunderbird Lighting 1.0pre
- contains iCal VTIMEZONE component
- also contains not yet used latitude and longitude for timezone
- methods to convert between TZID string, nummeric tz_id and VTIMEZONE

(using tz_id as foreing key into egw_cal_timezones table)
