Remove lazy records (#12682)
# Description
Removes lazy records from the language, following from the reasons
outlined in #12622. Namely, this should make semantics more clear and
will eliminate concerns regarding maintainability.

# User-Facing Changes
- Breaking change: `lazy make` is removed.
- Breaking change: `describe --collect-lazyrecords` flag is removed.
- `sys` and `debug info` now return regular records.

# After Submitting
- Update nushell book if necessary.
- Explore new `sys` and `debug info` APIs to prevent them from taking
too long (e.g., subcommands or taking an optional column/cell-path
argument).

Fix replacement closures for update, insert, and upsert (#11258)
# Description
This PR addresses #11204 which points out that using a closure for the
replacement value with `update`, `insert`, or `upsert` does not work for
lists.

# User-Facing Changes
- Replacement closures should now work for lists in `upsert`, `insert`,
and `update`. E.g., `[0] | update 0 {|i| $i + 1 }` now gives `[1]`
instead of an unhelpful error.
- `[1 2] | insert 4 20` no longer works. Before, this would give `[1, 2,
null, null, 20]`, but now it gives an error. This was done to match the
intended behavior in `Value::insert_data_at_cell_path`, whereas the
behavior before was probably unintentional. Following
`Value::insert_data_at_cell_path`, inserting at the end of a list is
also fine, so the valid indices for `upsert` and `insert` are
`0..=length` just like `Vec::insert` or list inserts in other languages.

Added tests for `upsert`, `insert`, and `update`:
- Replacement closures for lists, list streams, records, and tables
- Other list stream tests

Remove dead tests depending on inc (#10179)
They relied on the `nu_plugin_inc` but where behind a feature flag that
isn't actually defined anywhere. These tests of `update` or `upsert`
shouldn't really depend on `inc` so I decided to remove them outright as
they haven't been used to exercise the commands under test.

This PR is related to **Tests: clean up unnecessary use of cwd,
pipeline(), etc.

- Removed the `r#"..."#` raw string literal syntax, which is unnecessary
when there are no special characters that need quoting from the tests
that use the `nu!` macro.
- `cwd:` and `pipeline()` has not changed

make insert, update, upsert support lazy records (#9323)

Fixes: #9165
It's because `sys` returns a lazy record, and `insert`, `update`,
`upsert` can't operate on lazy record yet.

Slim down tests (#9021)
This PR just tidies up some tests by removing unused code:

1. If the filesystem is not touched, don't use the filesystem
playground/sandbox
2. If the filesystem is not touched, don't specify the `cwd`
3. If the command is short, don't bother wrapping it in `pipeline()`
4. If the command doesn't have quotes, don't bother with a `r#"..."#`
raw string

Part of #8670.

Restrict closure expression to be something like {|| ...} (#8290)

when define a closure without pipe, nushell will raise error for now:

`any`, `each`, `all`, `where` command accepts closure, it forces user
input closure like `{||`, or parse error will returned.
```
❯ {major:2, minor:1, patch:4} | values | each { into string }
Error: nu::parser::closure_missing_pipe

`with-env`, `do`, `def`, `try` are special, they still remain the same,
although it says that it accepts a closure, but they don't need to be
written like `{||`, it's more likely a block but can capture variable
outside of scope:

❯ def test [input] { echo [0 1 2] | do { do { echo $input } } }; test aaa
aaa
```

Just realize that It's a big breaking change, we need to update config
and scripts...

Remove deprecated --numbered flag from four commands (#7777)
# Description

Remove `--numbered` from ~~`for`~~, `each`, `par-each`, `reduce` and
`each while`. These all provide indexes (numbering) via the optional
second param to their closures.

EDIT: Closes #6986.

Every command that had `--numbered` listed as "deprecated" in their help
docs is affected.

Improve empty pipelines (#7383)
# Description

This fix changes pipelines to allow them to actually be empty. Mapping
over empty pipelines gives empty pipelines. Empty pipelines immediately
return `None` when iterated.

This removes a some of where `Span::new(0, 0)` was coming from, though
there are other cases where we still use it.

Allow iteration blocks to have an optional extra index parameter (alternative to -n flags) (#6994)
Alters `all`, `any`, `each while`, `each`, `insert`, `par-each`, `reduce`, `update`, `upsert` and `where`,
so that their blocks take an optional parameter containing the index.

nu-cli refactor moving commands into their own crate nu-command (#2910)
